  • A statement under penalty of perjury that you have a good faith belief that the material was removed as a result of mistake or misidentification.
  • A statement that you consent to the jurisdiction of the federal court in your district (if you are located in the United States) or the equivalent legal body in your country, and that you will accept service of process from the party who submitted the original DMCA notice.
  • Your physical or electronic signature.

3. Repeat Infringers
